Inter-County Health Plan customers now have access to Connections® Health Management Programs providing support and information to help members take an active role in managing their health and making informed health decisions. Connections encompasses disease management, decision support, health coaching, outreach and educational resources. The program addresses a wide spectrum of health issues including chronic health conditions, as well as general health information. Inter-County provides Connections as a health plan enhancement, at no cost to members or their employers.
Using Connections’ many resources, members with chronic health conditions such as diabetes, heart, vascular and pulmonary disease can learn more about their condition and develop a plan to maintain a healthy lifestyle. Connections can increase members’ knowledge, aid their decision making, and help them interact with their physicians when they are faced with significant medical events. Connections also helps members manage everyday health concerns, providing a convenient resource for general health information.

Plan members can access Connections resources around-the-clock online as well as through a toll-free phone number. Connections also sends members program communications and provides a wide range of outreach materials for members with health conditions.
Connections personal health coaches, available anytime day and night, are one of the program’s highlights. Once a member connects with a health coach, they can interact with that same coach over and over, and there are no time limits to the number of calls members can make or the length of their conversation. Connections health coaches are experienced, licensed healthcare professionals. They offer the support members need to take an active role in their healthcare, strengthen their relationship with their doctor and be better prepared to make informed healthcare decisions.
Connections also features the Dialog CenterSM online tools and resources, a secure environment where members can access a wide variety of health information and decision support tools, check for health coach messages, track medications and symptoms, and take a health survey. Other key resources available through the
Dialog
Center
are the Healthwise® Knowledgebase, an online encyclopedia covering thousands of health topics, Healthwise HEAR® Audio Library and ConnectionsSM health videos.
Inter-County is providing members with Connections Health Management Programs as part of its consumer-driven initiative designed to empower individuals with decision-making tools and resources that will enable them to be more proactive in their healthcare and optimize their use of healthcare resources to get the greatest benefit. In addition to comprehensive major medical plans, Inter-County offers HealthAccount+ consumer-driven health benefits plans including Flexible Spending Accounts (FSAs); Health Reimbursement Arrangements (HRAs); HSA-compliant high deductible plans; online resources for health account management; and the AmeriHealth+ debit card. HealthAccount+ consumer-driven programs are administered by AmeriHealth Administrators, an affiliate of Inter-County Health Plans.
