Blood Glucose Meter Program
Inter-County is pleased to offer you the convenience of online access to your health plan information and resources to help you manage your health, provided by AmeriHealth Administrators.
AmeriHealth Administrators’ pharmacy benefits management company, FutureScripts®, offers an expanded Blood Glucose Meter Program with a free Abbott or Bayer testing system for patients who are currently using another brand of blood glucose meter or are new to blood glucose testing.
Preferred Test Strips
- For Abbott monitors, the preferred test strips include FreeStyle®, FreeStyle Lite®, and Precision Xtra®.
- For Bayer monitors, the preferred test strips include Contour®, Breeze®2, Elite®, and Autodisc®.
Prior Authorization Information
If you are enrolled in a formulary program that requires prior authorization and you choose to use a test strip other than FreeStyle®, FreeStyle Lite®, Precision Xtra®, Contour®, Breeze®2, Elite®, or Autodisc®, your doctor can submit a request for prior authorization. If approved according to the program’s criteria, your prescription will be covered at the higher, non-formulary cost-sharing level. If denied, the test strips will not be covered, and you would need to pay the full cost in order to continue using them.
Free Blood Glucose Meters
Abbott and Bayer blood glucose meters are available at no cost to you. The following chart compares important features of the blood glucose meters and corresponding test strips. These preferred brands rank high in patient satisfaction and can be used in several test sites, as listed below.
